Does the term "wahlärztlich" in the expansion of the acronym IWA, "Inkassostelle für wahlärztliche Leistungen" refer to services rendered by out-of-plan doctors chosen by patients, similar to situations with U.S. HMOs, or is there something else behind the terms "wahlärztlich" and the related "Wahlarzt"?

Explanation:
HMO plans with a limited number of health care providers are history in Germany, where everybody is granted the free choice of a physician by law, however, there are physicians (Wahlaerzte), who are not registered with the "Kassenaerztliche Vereinigung" (National Health Insurance Scheme), and whose patients are considered "private" patients. The physicians registered, are so-called "Kassenaerzte". There are basically two types of health insurances, the "Krankenkasse" (Health Insurance Fund/Agency) and the "Ersatzkasse" (Substitute Private Health Insurance). Everybody has the free choice of insurance type, no matter what the employer would prefer, but may face problems if he tries to switch from one type to the other more than once. "Ersatzkassen" policies are in most cases more expensive than those of Health Insurance Funds, but they usually cover also treatments (full or partially) with physicians, who are not registered with the "Kassenaerztliche Vereinigung".
