English translation:illness of the (horse's) spavin
Explanation:
Spat=an infection of the horse's hock, according to Wahrig's Deutsches Wörterbuch, thus Spatenerkrankung would be and infection or illness of the hock. I also found the following defintion for "Knochenspat" on the website <http://www.microtec.net/bouletjc/vc2e/searvc2.htm>, the Equine Multilingual Dictionary:
"Osteitis or osteo-arthritis of upper end of canon and inner side of hock. A large spavin is called a jack, and a small is a blind or occult spavin. The latter may present no palpable or radiographic sign."
