German term or phrase: Nachbefundung
Nachbefundung einer auswärigen Untersuchung
S.Groth
English translation:follow-up diagnosis
Explanation:
this a bit of patchwork :

Nachuntersuchung = follow-up examination
nachuntersuchen = to post-examine
(Unseldt, Med. Wörterbuch)

Befund = Diagnose
(Deutscher Duden, Synonyms)

follow-up findings, post-diagnosis ... ?
both don't sound good to me.
Posterior refers to locality, not to time, It's not right.

possibly also "rescreening"

Explanation:
I agree with Uschi's answer; "follow-up diagnosis" sounds very plausible.

In a cytological/histological context, I also found "rescreening", cf. below:
www.vdca.de/wir.htm

If your context is not specifically oncology/histology/cytology, I'd go with Uschi's answer; just thought I'd add this to round out the picture.
Good luck.
