The text is discussing the results of various medical studies and, in particular, whether testosterone has an anti-ischaemic effect. The author writes that it does but adds 'dies muß allerdings noch mit größeren Kollektiven' und "harten Endpunkten" (z.B. Infarktinzidenz, Mortalität) bestätigt werden.' How can I translate "harten Endpunkten"?

"End points are measures of health such as death or occurrence of an infarct. They form a subset of outcomes. They are more scientific than outcomes but not necessarily more attractive. Happiness is more of an outcome than an endpoint. End points are assessed in terms of debatability and are referred to on a scale of soft to hard. Death for example is an endpoint that is fairly 'undebatable' and relatively easily measured it is therefore a hard endpoint. Satisfaction is more difficult to measure and the results more debatable. It is a soft end point. Studies with hard end points are often more communicable and better received. This is because of the scientific nature of our speciality. Hard end points are not always appropriate however and may be unobtainable. Any study of my mothers baking would be badly reviewed by her if it looked at hard end points such as death and morbidity. Much better to look at soft end points such as satiety or mouth watering."

Doch unzulässige Verallgemeinerungen und unterschiedliche Methoden kennt nicht nur die klinische Forschung, das kennen andere Wissenschaften auch. Ein spezifisches Medizinerproblem ist jedoch das der "harten und weichen Endpunkte". Das heißt: Viele Arzneimittelstudien messen wohl die Wirkung des zu untersuchenden Medikaments auf ein bestimmtes Organ ("weiche Endpunkte") , nicht aber dessen tatsächliche Heilwirkung auf eine Krankheit ("harte Endpunkte"). Beispiel: Herzschwäche. Hier existieren zahllose Studien dazu, wie bestimmte Medikamente die Leistung des Herzmuskels verbessern. Aus diesen Ergebnissen wird dann der Schluss gezogen, dass die betreffenden Mittel auch bei Herzschwäche lebensverlängernd wirken.
