It's the voll zugelassenen part that I don't understand. The text is a reference for a doctor that worked "in der voll zugelassenen Ambulanz"

I don't think this is going to be too helpful I'm afraid. zugelassen = as you probably know usually means licensed / registered. eg a vehicle may or may not be licensed to carry passengers, but to be honest I don't know the system - does an ambulance have to have some sort of licence to carry patients????
