Reference information:
Bei einer Pleurakuppenschwiele handelt es sich um eine benigne tumorartige Pleuraläsion, die in der Regel Folge einer chronisch entzündlichen, ischämischen oder degenerativen Gewebsschädigung mit chronischer Atelektase und Elastose des restlichen parapleuralen Lungengerüstes ist (Abbildung A, Kapitel 4.2.2.4) (Müller 1983). Bei einer Pleuradicke von mehr als 0,4 mm handelt es sich in der Regel um eine Schwiele. Makroskopisch imponieren diese z. T. bis in den Zentimeterbereich verbreiterten Areale als stark fibrös verbreiterte Pleura mit häufig sekundärer Verkalkung und Verknöcherung und in Einzelfällen mit einem Übergriff auf angrenzende Gewebsstrukturen wie die Brustwand. Histologisch ergibt sich ein teils geordnetes, teils ungeordnetes kollagenes Bindegewebe, welches häufig stark vaskularisiert ist und Zeichen einer entzündlichen Restinfiltration aufweist. Kausalpathogenetisch lassen sich die Schwielen hauptsächlich nach a) Hinweis auf eine abgelaufende Tuberkulose, b) Kombination mit einem Mischstaubgranulom oder c) isoliertem Auftreten einer ‚Kuppenschwiele’ ohne konkrete kausale Ableitung, auch „idiopathische“ Kuppenschwiele genannt, einteilen (Brockmann und Stolpe 1991).
