Bei der Verdachtsdiagnose einer autoimmun verursachten Encephalopathy handelt es sich somit um eine Verdachtsdiagnose. Eine Sicherung der Diagnose durch den Nachweis spezifischer Antikörper, typischer MRT-Veränderungen oder einen auffälligen Liquorbefund was bisher nicht möglich. Der fehlende positive Nachweis schließt aber trotzdem eine Autoimmunerkrankung nicht aus. Die Sicherung der Diagnose kann in so einer Situation letztendlich nur durch das potentielle Ansprechen auf eine Therapie erfolgen. Da es sich bei der Patientin vor allem um Symptome im Bereich der Kognition und des Verhaltens handelt, ist ein Therapieansprechen nicht ganz einfach zu objektivieren. Ich würde vorschlagen, vor einem erneuten Therapieversuch mit Immunglobulinen mit der behandelnden Psychologin und den Lehrern ein umschriebenes Zielkriterium für einen Therapieerfolg festzulegen. Vielleicht lässt dies im Rahmen einer beschränkten kognitiven Testung auch objektiv festhalten. Eine solche reduzierte Testbatterie könnte dann vor einer erneuten Therapie mit Immunglobulinen durchgeführt werden und 2 Wochen nach der Therapie wiederholt werden.

With Lancashireman's input in the discussion, I realized that "objectiv festhalten" is most likely another way of saying "festlegen" or "festschreiben." "Dies" is, then, best anchored to the text if it refers to "ein umschriebenes Zielkriterium."

To me (Ausländerin), the structure and diction appear nonstandard. This is what I think each sentence essentially means.
1. Clinical suspicion of encephalopathy linked to autoimmune disease or autoimmune encephalopathy. (Not sure about entire sentence, since Verdachtsdiagnose is repeated, but some type of Diagnose fits.)
2. Biochemical testing and MRI to confirm diagnosis did not find anything wrong. (Notice that "fehlende positive" is not the technical term "false positive" and should be completely reworded or omitted.)
3. This does not mean that the patient does not have an autoimmune disease.
4. Now, response to a potential therapeutic treatment is the only means of getting a sure diagnosis.
5. With this solution, there is a problem: picturing/gauging the response to therapy is not easy. (Better may be "quantifying the effect of therapy.")
