I read the following quote to mean an NHS doctor acting privately:

'Ein Privatarzt oder Wahlarzt ist ein niedergelassener Arzt, der nicht in einem Vertragsverhältnis zur Krankenkasse seines Patienten steht. Er kann gar keine Kassenverträge haben, oder nur solche mit bestimmten Kassen'
