My draft:
The participants were asked about their most recent outpatient treatment of a patient with 1 of the 3 symptom profiles "panic disorder with agoraphobia", "panic disorder without agoraphobia", or "agoraphobia without a history of panic disorder" as the main diagnosis, and to answer the question of whether a therapeutic rationale mediates [[what?]] and which of the 4 possible means of delivery of exposure were used.

This doesn't make sense to me, but if the queried phrase means "whether a therapeutic rationale is mediated" (or taught or conveyed or required), then it would have to be in passive voice, which it isn't.
