Hebrew term or phrase: תשובה
In a hospital discharge form, after headings on allergies and examination on admission comes this heading which is followed by a description of what was done on admittance. Would this be the "response" of the medical team or something else?

In the usual Discharge Summary, after the Physical Examination come the
lab reports/results.

many hospitals use formats and the MD dictating the DS just fills in the blanks
or replaces old text with new text. In many hospitals the MD dictates an Admission Note on admission and a DS at discharge, using the AN material in the DS, and just
adding or deleting text and info, accordingly. If the MD is not careful (or too tired),
this can lead to "undeleted text fragments" dangling at odd places in the DS. It can be very confusing and sometimes even funny.
