Does it always necessarily mean he started playing it, or only that he busied himself with it in some way?

When used in reference to a person sitting down at an instrument (usually used about a stationary instrument like a piano) it means that the person started playing the instrument. If he had sat down by it without playing it, it would have been "settist hjá" or "settist með" - or "settist við" plus words to the effect that he did not play it.
