Italian term or phrase: dichiarazione di conoscenza
Io sottoscritto XY sindaco di XY dichiaro di conoscere personalmente il Sig. XY...
(ai fini ottenimento visto australiano)

This is literally a declaration/statement of knowledge of a third person...

Is there a formal way to say it?

Thanks!

declaration of acquaintance/confirmation of identity
Explanation:
Take your pick. You basically need a title that summarises what the letter says. You can either stay close to the Italian, as with my first option, or provide a slightly looser (and I think more natural-sounding) version.
