no need to speak of (or to specify) the medical condition, imo, since it's clear in the context

he è lo specialista ospedaliero... in italia non è il paziente a compilare la application form... che non è neanche una vera e propria form, ma più un certificato... ma un certificato nel senso di una dichiarazione, un'attestazione, perché il certificato valido per l'esenzione viene dopo... io, per chiarezza e per localizzazione, parlerei di "form" e lascerei "certificate" all'esenzione vera e propria (quella che ti richiedono il farmacista o l'ospedale al posto dei soldi)

btw, non è che in italia tutte le malattie croniche permettano di non pagare mai nulla al sistema sanitario... ci sono tabelle e regolamenti molto specifici e, per quanto ne so io, fin troppo restrittivi...

Explanation:
In Italy, the whole range of chronic ilnesses absolves one from paying the ticket for medical services/vists, etc.. Cancer, for ex.; all kinds of cario-vascular; mental illnesses, etc.

That's what I think it is. Unless, of course it is exemption from the military service (but there's no longer any draft in Italy, so I doubt it)

agree  SJLD: "exemption from copayment" perhaps?/copayment is the term used in the US too for the out-of-pocket contribution paid by the patient

  -> thanks! "copayment" is outside my sphere of competence: must be British-system term for what in North America (and Italy!) is usually referred to as "ticket".thanks. my lacuna: never came across that.

he è lo specialista ospedaliero... in italia non è il paziente a compilare la application form... che non è neanche una vera e propria form, ma più un certificato... ma un certificato nel senso di una dichiarazione, un'attestazione, perché il certificato valido per l'esenzione viene dopo... io, per chiarezza e per localizzazione, parlerei di "form" e lascerei "certificate" all'esenzione vera e propria (quella che ti richiedono il farmacista o l'ospedale al posto dei soldi)
