normal postop Explanation: as opposed to abnormal (non integra) -------------------------------------------------- Note added at 11 hrs (2013-12-28 10:13:23 GMT) -------------------------------------------------- Maybe you need to add care (normal postop care) and the non integra would involve "postop complications" http://www.healthline.com/health/postoperative-care This is not "my" field... |

All excisions include a simple closure as part of the surgical package, and therefore, may not be billed separately. However, for excisions that require more than a simple closure, coders can report either an intermediate (12031-12057) or complex (13100- 13160) repair, in addition to the excision. Wound repair codes should only be used when the physician uses sutures, staples, or tissue adhesives to close a wound. If a physician only uses adhesive strips to close a wound, the repair must be reported using an E/M code (99201-99499) instead. The following steps will help you to code for a wound repair: Step 1 – How was the wound closed? § Adhesive strips only (See 99201-99499.) § Sutures or staples, with or without tissue adhesives (See 12001-13160.) Step 2 – What type of closure did the physician perform? § Simple - A single layered closure of the skin (epidermal or dermal) or subcutaneous tissues (Report no additional wound repair code if it is performed as part of an excision.) § Intermediate - A layered closure of one or more of the deeper layers of subcutaneous tissue and (non-muscle) fascia, plus a single layered closure of the skin (epidermal or dermal) (See 12031-12057.) § Complex - A repair that requires more than a layered closure to close the wound. The additional work may include a scar revision, debridment (for complicated lacerations or avulsions), extensive undermining, stents, and/ or retention sutures (See 13100-13160.) - See more at: http://codapedia.com/article_502_Coding-Excisions-and-Wound-... -------------------------------------------------- Note added at 20 hrs (2013-12-28 19:54:37 GMT) -------------------------------------------------- I think here medicazione is a service http://vitalsalus.it/prestazioni.html I'm not sure medication has this meaning, it implies use of drugs/ dressings but also management of the problem. Care would be fine but how can you put it in a list? wound care? -------------------------------------------------- Note added at 21 hrs (2013-12-28 20:05:48 GMT) -------------------------------------------------- medication: the act or process of medicating http://i.word.com/idictionary/medication it should be ok -------------------------------------------------- Note added at 1 day11 hrs (2013-12-29 10:44:31 GMT) -------------------------------------------------- http://books.google.it/books?id=t4Ut3mTJdjcC&pg=PA258&lpg=PA... |

conservative post-operative Explanation: http://books.google.co.uk/books?id=qAfnmOhENQ8C&pg=PA758&lpg... -------------------------------------------------- Note added at 1 day11 hrs (2013-12-29 10:53:08 GMT) -------------------------------------------------- By the way, just to make it clear "medicazioni" = dressings http://www.salute-italia.it/dispositivi/m0402 -------------------------------------------------- Note added at 1 day11 hrs (2013-12-29 10:57:05 GMT) -------------------------------------------------- http://www.rcn.org.uk/__data/assets/pdf_file/0007/109843/002... Stag e 1: Pressure ulcer is defined as an erythema of the intact skin. The reddened area remains red after pressure is relieved. Key features include: persistent discoloration of the skin, including non-blanchable erythema on light skins and blu e-black discoloration on darker skins. Stag e 2: Pressure ulcer is defined as partial thickness skin loss involving epidermis or dermis. The ulcer is superficial and presents clinically as an abrasion, blister or swollen crater -------------------------------------------------- Note added at 1 day12 hrs (2013-12-29 10:59:17 GMT) -------------------------------------------------- http://sci.washington.edu/info/pamphlets/pressure_sores.asp STAGE ONE Stage One How to recognize: Skin is not broken but is red or discolored. The redness or change in color does not fade within 30 minutes after pressure is removed. What to do: Keep pressure off the sore! Maintain good hygiene. Wash with mild soap and water, rinse well, pat dry carefully (but gently). Do not rub vigorously directly over the wound. Evaluate your diet - are you getting enough protein, calories, vitamins A and C, zinc and iron? All of these are necessary for healthy skin. Review your mattress, wheelchair cushion, transfers, pressure releases, and turning techniques for possible cause of the problem. If the sore seems to be caused by friction, sometimes a protective transparent dressing such as Op-Site or Tegaderm may help protect the area by allowing the skin to slide easily. If the sore does not heal in a few days or recurs, consult your health care provider. STAGE TWO Stage two How to recognize: The epidermis or topmost layer of the skin is broken, creating a shallow open sore. Drainage may or may not be present. What to do: Follow steps 1-4 under Stage One. Consult your health care provider for further treatment, which may include the following: 5. Cleanse the wound with water or saline solution and dry carefully. Apply either a thin foam dressing (such as Allevyn), a hydrocolloid dressing (such as DuoDERM), or saline dampened gauze. The first two types of dressing can be left on until they wrinkle or loosen (up to 5 days). If using gauze, it should be changed twice a day and should remain damp between dressing changes. 6. Check for signs of wound healing with each dressing change. 7. If there are signs of infection (see Signs of Trouble , below), consult your health care provider for alternative wound care ideas and review of possible causes (see step 4 under Stage One). -------------------------------------------------- Note added at 1 day12 hrs (2013-12-29 11:01:00 GMT) -------------------------------------------------- So dressing used for stage 1 pressure sore/ulcer = sometimes a protective transparent dressing such as Op-Site or Tegaderm may help protect the area and dressing/s used for stage 2 pressure sore/ulcer = Apply either a thin foam dressing (such as Allevyn), a hydrocolloid dressing (such as DuoDERM), or saline dampened gauze.
