i think i'm pretty sure that "dichiarazione di certificazione" can be translated as "statement of certification", but i'm not sure about the "sostitutiva" at all!

i haven't found anything on the internet...do you have any idea?

agree  Marie-Hélène Hayles: yes - the "sostitutiva" here means "invece di". I'd go with "self-certification".
