Italian term or phrase: cp (not tablet)
This is an abbreviation used in a patient's medical records referring to heparin dosage during dialysis. It may be a typo (there are lots of them in this document):

Presenza di coagulo nel pozzetito venoso. Aumenta cp continua a 3500, ripete TC venerdì e lunedì

I think it’s an injectable dosage of 3500 units taken from a vial.
