Explanation:
I would limit it to an "eye-bolt" An engineer collegue of mine said that it was "una specie di anello dove scorre una corda e fissato in qualche modo al sommazzatore." If it were a type of hook, the rope could then come out. "A type of ring or "eye-bolt" used as a fixed guide for something like a rope or wire."

Explanation:
I can't find any direct translation but from the text below it appears to be some kind of hook or eye-bolt:
"Il sommozzatore ha il polso sinistro collegato con una sagola alla pietra e con una ridancia entro cui la cima scorre libera. Scendendo, tiene la pietra stretta fra le mani, come un timone di direzione, l'adagia sul fondo per poter lavorare e avere comunque un punto di riferimento e di ancoraggio; per risalire, dà uno strattone e si assicura alla pietra che gli amici, dalla barca, recuperano a grandi bracciate. Da quanto si capisce non compensa. Non indossa né maschera né pinne".
