I couldn't find any equivalent for someone who is extremely annoying and tortures you mentally. Thanks in advance.

mental tormentor/scourge
Explanation:
I think this if very close to what you want...normally I would use "he/she grates on my nerves" but you want something stronger

It is not a "phrase" like سوحان دوح but everyone will understand it. It fits perfectly with your explanation in the entry below. Tormentor is better than scourge.

I don't believe we have anything equivalent in English. I always thought it was a great expression when I first learned it. The imagery is very expressive.

Pain in the neck

Explanation:
.

For example, he is a pain in the neck.

There are a number of expressions that are used only for humans. Nudnik may be closer to what you are looking for.

--------------------------------------------------

In that case, I would say "He is a real pain in the neck." There are also vulgar variations that may be stronger in emphasis.

Asker: Thanks but this expression doesn't necessarily apply to a human being. Jobs or traffic can be a pain in the neck. I'm looking for an adjective with almost the exact same depth that applies to humans.

Asker: I understand but imagine this scenario: I'm looking to describe someone who repeatedly hurts me mentally and is very foul-mouthed and an addict and has very high expectations of me and always wants me to do stuff for him as if it's my duty etc.

Notes to answerer
Asker: Hi. Thanks for you answer but I don't think this expression quite conveys the hatred that the Persian equivalent carries. I had seen that expression before. I'm not looking for an expression, but an adjective.

Asker: The phrase سوهان روح goes way deeper than that. "Nuisance" is closer to مزاحم or موی دماغ or "اعصاب‌خوردکن". This phrase is defined as "آزاردهنده ٔ جان" and I think the following describes it perfectly: "سوهان روح" اصطلاحی است که گوینده برای بیان نهایت آزرده شدن روح و روان خود توسط دیگری، بکار می بندد. کسی که با رفتار و یا گفتار خویش، ذره ذره جان مخاطب را می تراشد و له می کند.

Asker: Would you translate "he's a pain in the neck" to "اون سوهان روح منه"?
