D. (medical doctor) chooses specialization by going into a residency program. Following residency (about 5 years), the doctor may go for an additional FELLOWSHIP year or two for DEEPER specialization. The definition of Residency varies worldwide by country and structure of the medical industry. Residencies are also available, and may be required, for students graduating from pharmacy and physical therapy schools. In the US, the training of podiatrists, optometrists, and dentists may also involve a residency period. A residency may follow the internship year or include the internship year as the first year of residency. The residency can also be followed by a fellowship, during which the physician is trained in a specialty or a sub-specialty. The first year of training after medical school is called internship, or more commonly it is called first year of residency or PGY-1 (PostGraduate Year-1). The following years are called PGY-2, PGY-3, etc. The training that is done after a residency (in a subspecialty) is usually called a fellowship. Much of what you will learn in your chosen specialty will be learned in your residency.
