Praca przedstawia przypadek 22-letniej pacjentki, u której wystąpiły zaburzenia psychiatryczne jako pierwsze objawy choroby Wilsona (chW). Pacjentkę z rozpoznaniem schizofrenii paranoidalnej przeniesiono do kliniki psychiatrii z powodu niepowodzeń terapeutycznych w ciągu ośmiu miesięcy trwania choroby. Przy przyjęciu uwagę zwracały nasilone objawy pozapiramidowe: wzmożona sztywność mięśniowa, drżenie głowy oraz kończyn, przymusowe zgięciowe ustawienie kończyn górnych, hipomimia, dysfagia, ślinotok, dyzartria oraz spłycony afekt.

The article describes a 22-year-old woman who presented with neuropsychiatric symptoms in the course of Wilson’s disease. The patient, diagnosed with para- noid schizophrenia, was transferred to a university hospital’s psychiatric ward due to treatment failure in the past eight months of progressing disease. On admission the patient showed severe extrapyramidal symptoms: rigidity, tremor, dystonia, dysphagia, sialorrhoea, masked facies, flat affect, and dysarthric speech.

Flat affect: A severe reduction in emotional expressiveness. People with depression and schizophrenia often show flat affect. A person with schizophrenia may not show the signs of normal emotion, perhaps may speak in a monotonous voice, have diminished facial expressions, and appear extremely apathetic.
