I was the asker, so I should not give any credit myself. However I think that my suggestion is the most acceptable in the USA. I am open to any suggestions or explenations.

Professor doctor

Explanation:
There is no such title or degree in the English or American education system as 'habilitation'. We used to call a doctor with habilitation a docent. In English language a doc is an informal word used while adressing a medical or university doctor. Thus, if you want to put the titles 'profesor doctor habilitowany' in front of the name, you just put: 'Prof. dr.'

Professor, First and Last Name, Ph.D.

Explanation:
Because there is a different process of obtaining titles in science in the USA and Poland. My suggestion will compromise this differences and satisfy the holders of the title.
