"Para constar"= for the records
"se lavrou a presente acta" = these minutes (of the meeting) were written
"que lida e achada conforme" = which were read and found to comply with the facts
"vai ser devidamente assinada" = will be duly signed
The order of the expressions should be slightly changed in English, and it should read:
"These minutes (of the meeting) were written for the records, which were read and, found to comply with the facts, will be duly signed (by the attending people)."
