It seems to me that the meaning of 'foro' here is number 7.
So I would translate it as:

preserve
'a domain that seems to be specially reserved for someone; "medicine is no longer a male preserve"'

DAPA is fundamentally medical preserve...

if you don't like to leave 'disease' out, you could say,

DAPA is fundamentally a disease for the medical forum...

which is more literal, although I prefer my first suggestion.
