The earlier answers used "fill" and "fill up" - if you want to use "fill" it must be fill OUT.

Do not insert any info as to which consulate is involved. You have no way of knowing, based on the original text. Especially don't write Brazilian with lower-case b.

if you say "have them notarized and certified at the consulate" then it seems as tho' the notarization is done at the consulate, which it is not.
