Portuguese term or phrase: em nosso meio
I see this in various Brazilian medical papers written by doctors and or researchers, e.g. "a etiologia da doença é pouco conhecida em nosso meio".

Is there a recognized translation for this, or is it best to translate it as "in Brazil" or "in the xyz institution or hospital" depending on where the author's research relates to?

I think they are talking about the medical profession as a whole. These days it is unlikely that only one country in the world would be unaware of research being conducted in other countries. Depending on the context it could also be translated as something like, there is little information available in medical literature but this would not work for the example you have given here.

I agree with Carla (and the definition she cites) that it's being used in the abstract sense;. However, I think it would be safer not to specify the exact field because the original doesn't either. My sense is that it refers to the medical specialty in question or the entire medical profession. Researchers tend to know what people are doing in their own field but not so much in other fields of medicine.

This is so different that I'm going to break the rules and open up a new answer. In the example cited, you could capture exactly the same meaning by turning the sentence around and saying:

"We (in our field) understand very little about the etiology of the disease"
