I was always writing kazaKHstan and this is what the dictionary says. However, I have a Kazakhstan passport in front of me which says over and over KazaKstan.
Can you let me know if any of these is wrong?

It's a bit confusing for Russians though, isn't it? After all, a казак is not the same thing as a казах.

BTW, while all Ukrainian official acts use the word Kyiv, I have to write Kiev in my letter to another country if they aren’t aware of the change…

I think the matter is in transcribing the word on the basis of what is actually pronounced by the Kazakh.
A similar case is Kyiv (as transcribed from the Ukrainian language), whereas Kiev from Russian

Kazakhstan is used more often. If you want to be on the safe side then use Kazakhstan.

Please bear in mind that former Soviet republics do not always use English for their passports. The former Soviet Union used French to transliterate names. For example, my name Поташник was spelled as Potachnik in my Soviet passport. I had to officially change my name from Potachnik to Potashnik after I left Russia and moved to Australia where French spelling made no sense.

It looks wer have here quite a discussion! My opinion is based on what I have seen working at Tengiz field (Chevron, Kasakhstan) – we have been instructed about spelling of the country name by Chevron legal dep.
