1. А что, отлом кончика скальпеля является побочным явлением? Ну, обломился - и обломился. Убрали, взяли другой и поехали дальше. Или предполагается, что кончик остается там внутри навсегда? Как память

2. Может есть какой-то специальный термин для такого являения, как отлом кончика скальпеля?

Reading Mark's comment, I am throwing this out.

I suppose scalpel nicks could also be amgibuous enough to refer either to the results of the scalpel tip hitting other things besides what is intended to be cut and any scalpel fragments produced during the operation.
