"Report" concerns information detected and/or confirmed by the auditors, who are performing the audit, which the auditors report to company management, an internal matter of the company.
"Disclose" refers to the information derived from the audit about the company that is disclosed to the general public in the financial statements by the auditors who performed the audit.
This is a big difference.

"On the lookout for" is too colloquial in my opinion, and the first "flaw" should be "flaws". "Weaknesses" is the word often used in an audit context.
Also, there's no need to capitalise words as the Spanish does.
You could leave out "determinamos condiciones reportables" from that very long sentence, as they're effectively saying the same thing twice: "We found disclosable weaknesses which we believe it is important to disclose".

Correction:

In our review of the internal control system, we reviewed the supporting documentation of the Project’s expenditures and investments. We were on the lookout for any flaw in the supporting documentation, [accounting] systems and records. We confirmed some flaws in the compliance with the Project’s Operating Regulations; consequently, during the review of the Project’s documents and operating procedures we ascertained reportable issues that we considered important to disclose as they may affect the presentation of the Project’s financial statements.
