Not sure what is meant here:

El paciente presenta rasgos de personalidad del tipo obsesivo, con tendencia a la autoexigencia. Si bien al inicio del cuadro actual se encontraba en un ambiente nuevo, desafiante y sometido a exigencias en cuanto a su rendimiento académico, actualmente persiste con dificultades del sueño incluso encontrándose de licencia escolar.

Actually, Liz, I think "school break" would be something slightly different in Spanish. "Licencia" is really a leave of absence rather than "vacaciones" or a "receso", i.e., someone taking leave while most others are studying or working as normal.
