If this is actually written on the birth certificate itself, you might want to say "This short form certificate has been issued in accordance with the requirements for obtaining ..."

Like the short form of the birth certificate in the UK and elsewhere, a 'certificación en extracto' doesn't show details of the person's parentage. The Spanish authorities require the application for a DNI to be accompanied by a 'certificación en extracto' issued especially for this purpose.

The whole certificate is neither given nor photocopied, just a "nota simple" from the Registrar's Office telling you the page and volume in which such document was logged (This is the process for getting the DNI, and consists of a communication between the Registro Civil and the Policía Nacional).
