Actually María Patricia is right; ischemic cardiomiopathy is used specifically to refer to dilated cardiomyopathy of ischemic cause (in the US at least).

IN as is most probably refers to isquémico-necrótica. It would be a weird acronym for "infecciosa" and there are not many other possibilities here.
And so ischemic cardiomyopathy seems an accurate translation of miocardiopatía dilatada isquémico-necrótica.

Ischemic cardiomyopathy (CM) is the most common type of dilated cardiomyopathy. In Ischemic CM, the heart's ability to pump blood is decreased because the heart's main pumping chamber, the left ventricle, is enlarged, dilated and weak. This is caused by ischemia - a lack of blood supply to the heart muscle caused by coronary artery disease and heart attacks.
