There is (almost) always more context than this, and the more background information you give your colleagues, the easier it is to come up with a helpful answer. In this case, although it looks like you've gotten the correct answer, the context is things like what type of document (ie hospital discharge report, medical journal article), where in the report this phrase appears (ie, history, laboratory results), and the patient's symptoms or diagnosis.
