Explanation: It all depends on whether you are translating into British or American English or where it is actually going to. But in any case, for what it's worth, here's my two cents:
In Australia/Britain we'd say: a statutory declaration (stat dec in spoken English) for declaracion jurada, particularlt when it comes to taxes - as seems to be the case here. However, in legal contexts, you'll find "sworn affidavit" to be more in use.
Now for saldo a pagar: balance due or unpaid balance are very much the norm.
Finally valor agregado is value added as in IVA or VAT for value added tax.
