This is a transaltion about a claim that may be rejected (insurance) and the sentence in question goes as follows:
A su vez, conforme requerimiento efectuado Cemic el 18/07/2001 amplia la informacion, adjuntando carta manuscrita del reclamante que el 06/04/2001 solicitaba cobertura de asistencia especializada en domicilio en forma permanente...

The most likely translation seems to be:

Then, in response to the claim presented, CEMIC on 18/07/01 provided more information, and attached a handwritten letter from the claimant dated 06/04/01 asking for permanent specialist home support/asssitance/help.

However your original is badly punctuated, and we don't therefore know if Cemic is the recipient of the requerimiento, the body to which the requerimiento was made, or a third party providing information.

What is clear is that something is missing between "efectuado" and "Cemic"; and I think there are at least the following possibilities:
i) , (the most likely in my opinion, because the simplest - Occam's razor)
ii) ante (el)
iii) a(l)
iv) por (el)
...equally there may or not be a comma missing after the date before "amplia".

By the way iis this anything to do with the medical training and investigation programme in Argentina? Often knowing the country concerned tells all.
