I think this is your safest translation. Though I agree with the preceeding answer that it may refer to a nurse (in which case, you can say "infirmarer" or "infirmarian" in a birth certificate), it might well not be the case. That's why I would feel safer using just "intervening party".

So I would say:

Inervening party: Juana Pacheco, LC 77.888.999.
Address: XXXXX. Acts in virtue of the authorization hereby filed.

Explanation:
Because it is a Partida de Nacimiento it could mean the assistant to the Dr. (as in a nurse/helper) I am not absolutely sure. You will have to decide if it is so, or if it is the person who came to register the child, although I doubt it.

The dictionary says "participant", but I also agree with Galarza, it might be better to decide if it was a nurse or somebody acting as a witness of the document. Obra en virtud de: acting by virtue of/by reason of.
