Office Number c. oficial del registro civil = Registrar (simply put). C. stands for Ciudadano... H. Ayuntamiento Constitucional = H. stands for Honorable, you could render it as City Hall, Town Hall, or City Council, the excess wording (Honorable and Constitucional) aren´t really necessary in my opinion for translating this into English.
