Most TRUS procedures and biopsies are performed without any surface anesthetics; however, Xylocaine jelly or periprostatic block may be used. Recently, Pareek et al described a technique of periprostatic nerve blockade. They injected 2.5 mL of lidocaine on each side at the prostate base at the junction of the prostate and the seminal vesicle (using a 5-in 22-gauge spinal needle through the ultrasound probe). In a randomized, double-blind, placebo-controlled study, they showed significant pain control during and after biopsy. Alavi et al compared the efficacy of intrarectal lidocaine gel and periprostatic nerve block and concluded that the nerve block was superior for pain control. Using this technique, saturation biopsies, with up to 20 cores, could be performed.
