About ovar[i]ectomy ... yes, the spelling with the "i" is far more common. Oophorectomy is correct, but you must take your target audience into account, and I can assure you that women who are not doctors or maybe hosp. nurses will not know what it means.
...and what's more, if you got rid of that bracket, the "como minimo" would qualify the "lapso de un año" also ... just shows you how misplaced puncuation screws one up.... and it's easy to get a bracket misplaced when there are lots
I think the last round bracket is misplaced, it should come after the last square bracket, as the sentence goes ....if she is (proven to be) surgically sterile ...for at least a year or finished the menopause at least 12 months ago...
Haven't included links to save space, but one year is all over the web as a cutoff point for failure to conceive. So although the epxression is a little odd, I would agree with Dee that it's "at least a year".
Normal fertility has been defined as achieving a pregnancy within 2 years by regular unprotected sexual intercourse.  However, many define infertility as the failure to conceive after 1 year of unprotected intercourse.
Note that a formal definition of "infertility" is: The diminished ability or the inability to conceive and have offspring. Infertility is also defined in specific terms as the *failure to conceive after a year of regular intercourse without contraception*
