Context: a medical report (from Colombia), stating that a patient did not inform his new physician about certain symptoms he started suffering from several months before, because "el medico no esta bien documentado acerca de su enfermedad (es nueva para el medico)..."

Explanation:
"documentado" in this context refers to not being aware of/familiar with the disease. Later on it is explained that it was the first time the doctor had come accross it.
I believe "informed" can be ambiguous as it may lead to the conclusion that someone failed to informed the doctor when it's the doctor himself who failed to do his own research, at least for what I can gather from the context.
