I'm not sure you are following the right logic here.
Healthcare provider is a healthcare professional who has a primary contact with a patient. But there are also healthcare professionals who only have secondary or tertiary contact (for example, those who customize medical aids for patients according to healthcare provider's directions)

Professional evaluator can be a member of a monitoring/evaluating body, doing the evaluation of the quality of service provided by the professional in question.
