My first thought was "Procedure - Requesting (Physician?)", but there are obvious problems with that conjecture.
Although on first glance I answered Procedimiento/Medico peticionario , upon reading the original text and realizing the presence of neurologia I would agree with Helena in that proc is the abbreviation of procedencia, in this case the neurology service and I also agree with her in that M. Petic is médico peticionario , which has been redacted.
