I don't understand the sentence construction, and I'm not sure which antibiotic replaced which here. This is from the medical record of a patient being treated for pneumonia.

The complete sentence reads "le cambiaron azitromicina por levofloxacina 750 mg 1/dia x 10 dias".

Did the levofloxacin replace the azithromycin, or the other way around?
