Apologies for yest another abbreviation which has me stumped. The before and after phrases are:

AC: ritmics sin soplos. AP: MVC. Abdomen: blando, depresible, no doloroso a la palpación sin irritación peritoneal.

AP = Auscultación pulmonar
MVC = mumurillo vesicular conservada
In an English history, this would usually be put as "Breaths sounds: normal"
Similarly, AC = Auscultación cardiaca, normally put in an English history as "Heart sounds:"
