Lorena's excellent source confirms that in Bolivia doctors signing medical certificates must put the number of their "matrícula profesional" after their name. This is standard practice in many countries. So "M.P.F" here really must correspond to that.

"M.P." obviously stands for "Matrícula Profesional", but what about "F."? The solution to this is that in Bolivia the number of the matrícula profesional consists of a letter, followed by a hyphen and then some figures (three or four in all the examples I've seen). So although it's split over two lines as you've set it out in the question, it must be "M.P. F-xxxx", with xxxx being digits.

Here are a few examples to illustrate this:

Matricula Profesional: C – 2893"
