I assume all the terms are related to studies. That being the case: Plan Diferenciado = Differentiated Plan; Plan Electivo = Elective Plan; Concentración = Concentration. References: The first two see URL below.
Concentration is a term used (for example) when getting a Master Degree or PHD and refers that more courses are taken in a specific area of the fielñd of study; such as an MBA (Master of Business Administration) with concentration in Finance, or Marketing, or Production, Etc. I tend to believe the names in spanish were taken from USA English. Hope it helps.
