I get the impression it further qualifies the state of a medical condition (refraktär angina pectoris, refraktär parodontit) but the meaning isn't clear to me. The phrase in which it occurs is "Refraktär-tillstånd" regarding information provided by the pacemaker's microprocessor about the machine's status.
