I have translated the above as: was in a "de facto relationship with..."

I really doubt if I should include "with no registration of marriage" because 'de facto' encompasses this meaning.

This is from a custody order from the Juvenile and Family Court.

I agree that "de facto relationship with" covers the whole meaning of the source, and you don't need the extra part. That's what I'd go with.
