Source text - French Quand j'inspecte les contours de mon visage dans le miroir je me dis que je ne suis pas mal comme mec. Je nose même pas me comparer avec le troubadour qui a emmené avec lui mon ex et ma fille. Entre lui et moi c'est le jour et la nuit. Je suis grand, bien proportionné; lui c’est un nabot, on ne le voit pas quand il passe. Si on ne fait pas attention on peut lui marcher dessus ou on peut croire que c’est un animal à quatre pattes et sans queue. J’ai une petite moustache, je suis beau; lui on dirait un primate qui aurait raté de justesse sa mutation vers l’espèce humaine. Donc le surnom de L’Hybride que je lui ai donné lui va comme un gant. Quant à sa manière de s’habiller, c’est la catastrophe! Est-ce que c’est parce qu’on est artiste qu’il faut s’habiller comme ça? C’est du pipeau, je connais des artistes qui sont toujours bien sapés avec des lunettes noires et un éventail pour mieux frimer. Moi je ne rigole pas avec l’habillement, mes amis du Jip’s le savent, y compris Roger Le Franco-Ivoirien. C’est pas pour me vanter, mes costumes sont taillés sur mesure. Je les achète en Italie, plus précisément à Bologne où j’écume les magasins, m’arrêtant à chaque boutique le long des arcades de cette cité. Lorsque j’ai emménagé ici je ne savais pas où ranger tout ça. J’ai six grosses malles d’habits et de chaussures — pour la plupart des Weston en croco, en anaconda ou en lézard, et je possède aussi des Church, des Bowen et autres chaussures anglaises.
Si je suis toujours habillé en costard c’est qu’il faut , comme on dit dans notre milieu de la Société des Ambianceurs et des Personnes Elégantes, la SAPE, une invention de chez nous, née dans le quartier Bacongo, à Brazzaville, vers le rond-point Total, polémique à part. C’est nous qui avons exporté la Sape à Paris, qu’on ne me raconte pas le contraire surtout que les faux prophètes pullulent ces derniers temps dans les rues de la Ville lumière au point qu’il est désormais difficile de séparer le bon grain de l’ivraie.
On m’objectera sans doute que les Ivoiriens et les Camerounais sont aussi des Sapeurs. Voyons, ils ne s’y sont mis que bien après parce que, les pauvres, ils se plaignaient que leurs femmes nous couraient après. Alors, ils se sont imaginé que c’était à cause de nos Weston et nos vestes Gianni Versace. Pourtant lorsque ces Ivoiriens désespérés et ces Camerounais désemparés portent les mêmes vêtements que nous, le Sapeur congolais l’emporte grâce a sa touche inimitable, c’est pas du chauvinisme, c’est la dure réalité, et je n’y peux rien pour eux...

Translation - English When I look at the contours of my face in the mirror I say to myself not bad for a guy. I wouldn’t even begin to compare myself with the act who took with him my wife and daughter. Him and me; we’re like night and day. I am tall, well proportioned; he’s a midget. You would miss him when he passes by. If you’re not careful you would walk over the top of him. Or, you might think he is an animal on all fours without a tale. I am good looking with a small moustache; he is a primate; a missing link in the chain of evolution. So you see, Crossbreed, the nickname I have given him fits perfectly. As for the way he dresses; what a disaster! Just because you’re an artist, does it mean you have to dress like that? What a load of rubbish. I know artists who are always well turned out; black shades, and a fan; all the better for striking the right pose. Dressing is important to me, as my friends at Jip’s, including Roger from the Ivory Coast, know. I don’t mean to show off but my suits are tailor-made. I buy them in Italy, Bologna to be precise, where I delve through the boutiques, along the arcades of this city. When I moved here I didn’t know where to put all my stuff. I have six large trunks of clothes and shoes: mostly Westons in crocodile, anaconda or lizard skin and I also own some Churches, Bowens and other English makes.
If I’m always well turned out, it’s because you have to “keep up a certain standard”, as we say at the Society of Ambianceurs and Elegant People. The SAEP started off near the roundabout at the Total garage in Bacongo, our district, of Brazzaville. I am being serious. It is us who brought the SAEP to Paris and you can’t tell me any different. Although, recently, in the city of light, there are those who would try. It is hard to separate the chaff from the wheat.
Some will no doubt say that those from the Ivory Coast and Cameroon are also Sapeurs. Let’s see. The Ivoirians and Cameroons only started dressing like us after a while because they were complaining about their wives running after us. Poor sods. You see, they thought that it was because of our Westons and our Versace jackets. Yet when these confused losers wear the same as us, the Congolese Sapeurs, who bring our own unique, graceful style, they can’t match that. That is the way it is and there is nothing I can do for them.

Source text - French Qu’est- ce que c’est la trachéomalacie?
La trachéomalacie est une anomalie de développement de la trachée . La
malformation est définie par un déficit du support cartilagineux de la trachée, responsable d'une mollesse anormale de la trachée à ce niveau et d'un aplatissement. Le taux d’incidence est 1 sur 2100 nouveau-nés. Du fait de cette mollesse trachéale, le stridor du nouveau-né est caractérisé par un bruit inspiratoire ou expiratoire. Il est parfois permanent, quelle que soit l’activité de l ’enfant, éveillé ou pendant son sommeil. Dans d’autres cas, il est intermittent, se manifestant surtout à l’effort (toux, pleurs, tetée). Une fermeture complète de la trachée est alors observée, avec un bruit caractéristique, aboyant, simulant un cri de phoque.
Quels examens faire devant des symptômes compliquant une trachéomalacie?
Tout stridor doit être exploré par une endoscopie sous anesthésie générale. L’endoscopie bronchique ( bronchoscopie ) est l’examen de choix pour affimer le diagnostic de trachéomalacie et mesurer son intensitié; le degré de trachéomalacie. Cet examen se réalise sous anesthésie générale Les nouvelles techniques de scanner permettent des reconstitutions 3D de la trachée et peuvent constituer une approche diagnostique de la trachéomalacie. Toutefois, ces nouvelles générations de scanner ne sont pas encore disponibles dans tous les centres hospitaliers, et les renseignements fournis apparaissent moins complets qu'avec l'endoscopie qui permet la visualisation directe de la trachée.
Quels sont les symptômes secondaires à une trachéomalacie?
Parfois, la trachéomalacie peut engendrer un reflux gastro-oesophagien (RGO) et/ou les infections respiratoires répétées. Des troubles de la déglutition, responsables de fausses routes alimentaires. L’enfant a tendance à avaler de travers, pas toujours de façon spectaculaire, de sorte que les aliments peuvent descendre dans les bronches, les obstruer et les infecter. En dehors de ces fausses routes alimentaires, le stridor congénital peut aggraver une infection respiratoire telle qu’une bronchite ou une bronchiolite, surtout s’il s’agit d’un petit nourrisson. Les rares episodes graves d’obstuction trachéale complète sont décrits, avec cyanose et parfois perte de conscience.
Quelle est l’évolution de ces manifestations avec le temps?
Le stridor va disparaître progressivement pour la majorité des enfants à l’âge de 18-24 mois et chez eux ou il persiste, l’intensité diminuera clairement grâce à l’augmentation de la rigidité trachéale avec l’âge.
Quelle prise en charge?
La trachéomalacie se limitant à une respiration bruyante ne nécessite pas de prise en charge particulière. Lorsque des infections broncho-pulmonaires surviennent, la prise en charge varie selon la fréquence des manifestations. Des infections intermittentes, séparées par des intervalles sans aucun symptôme, vont en général ne nécessiter qu’une prise en charge ponctuelle, avec antibiothérapie. Les antiacides sont prescrits pour une persistance du reflux gastro-oesophagien.
Translation - English What is congenital tracheomalacia?
Congenital tracheomalacia is a weakness and floppiness of the walls of the trachea (commonly known as the windpipe) which is present at birth. The trachea is a tube supported by rings of cartilage. (Please see picture of trachea.) When it does not develop properly there is a softness in part of the cartilage which results in a lack of support of the trachea (windpipe). It is an uncommon condition affecting 1 out of 2100 newborns.
Due to this weakness in the trachea (windpipe) the new-born will have a stridor which means noisy breathing. Normally the trachea widens and lengthens slightly with each breath in and then on breathing out it relaxes again. To help you think of it more clearly, think of a straw that has a dent or floppy part in it. You notice that you cannot suck up juice properly, you have to suck harder and that it is noisy when you do so. This is similar to the stridor in new-borns with tracheomalacia. Their breathing is noisy and they have to work that bit harder. (Please see image of a normal trachea on breathing in and out and image with tracheomalacia).
Sometimes the stridor is always present, whether the child is awake or sleeping. In others, it is intermittent, getting worse when effort is involved (coughing, crying, feeding). Part of the trachea (windpipe) closes in and the noise of the stridor becomes high-pitched. It has been compared to barking or the cry of a seal.
What tests need to be done for the symptoms of tracheomalacia?
Any stridor has to be investigated by an endoscopy under general anaesthesia. The bronchial endoscopy or bronchoscopy is the examination of choice for the diagnosis of tracheomalacia, in order to measure the degree and intensity. This examination is performed under general anaesthesia. New scanners show the trachea in 3D and are another way to give a diagnosis. However, these scanners are not yet available in all hospitals and still do not give as comprehensive a picture as an endoscopy, which allows the trachea to be looked at directly.
What are the secondary symptoms of tracheomalacia?
Sometimes tracheomalacia can cause gastro-oesophageal reflux (acid reflux) and/or repeated respiratory infections. The child tends to swallow the wrong way which can cause choking. This is mostly not serious but particles of food can go down the bronchi, get clogged and cause infection. It can lead to bronchitis or bonchiolitis (chest infections), especially in small babies. Rarely, severe obstruction has led to cyanosis (blue colouring of the skin) and unconsciousness.
What is the long-term outlook for infants with a stridor?
The stridor will eventually disappear for the majority of children by the age of 18-24 months and for those where it persists, its intensity will noticeably lessen as the rigidity or strength of the trachea (windpipe) improves with age.
Aftercare
When a stridor (noisy breathing) is the only symptom of tracheomalacia no special medical care is required. If broncho-pulmonary infections (chest infections) occur, the care will vary depending on how often they happen. Intermittent infection with intervals in between where the child has no infection will normally be treated with antibiotics. Antacids are prescribed for persistent gastro-oesophageal reflux (acid reflux).
