Now, I often think about
how I was urged to drive faster.

I can see how only going
a couple of miles per hour faster

would be a minor risk.

But once I accepted that risk,
I would give up control over the outcome.

That outcome might have been
getting to the baseball practice on time

or earlier than we were expected,

and I suppose that, most of the time,
that would be the case.

But that outcome could have been
much more troubling.

It could have meant
losing the ability to slow down in time

to avoid a deer in the road
or to avoid a child chasing a ball.

It could have meant hurting myself,
hurting my children,

or hurting someone else's children.

That's painful to even think about,

but we need to think about it
to understand

why our commitment
to complete safety runs so deep.

And every time I hear someone question
why we're committed to complete safety,

I remember
the enormous consequences

a sad safety choice had on
one Cargill family a couple of years ago.

Cargill called, and they called me
at work at about 2:45

and just told me that
there had been a horrible accident

and that Scott
had been engulfed in grain.

And it's not just hard
because he was my husband.

It's hard because we have
two beautiful children

that will grow up without their daddy.

Once you go through a situation like this,

that culminated in this tragedy

And many are working through
the sad knowledge

in the chain of events

It's a sobering fact

I have read many near-miss reports.

What struck me over the years
is what a fine line there is

where there was a trip,

and there was little consequence
or perhaps a small scrape,

and then, in another similar case,
someone would trip

and hit their head on a pipe,
00:11:26:12 00:11:29:08
and we'd have
a significant lost-time accident.

They were such similar events,
such as a trip,
00:11:32:19 00:11:34:23
with such different outcomes.

This is why I believe
we must pursue solutions

in the areas of near-misses

with the urgency and discipline
we have around lost times

if we are to reach our safety goals.

I have this sense of urgency
because I felt the emptiness

and the discomfort of having
what would have been a near miss

turn into a tragic death
at one of the locations I once managed.

If the event had occurred
only 5 minutes later,

or if only 12 inches different in location,

it would have been a near-miss.

But, in this case, we had a tragic death
and a family and friends and teammates

to somehow comfort
and somehow answer to.

I felt so sad.

I'd not even been directly involved
with the location for over 15 years,

yet, when I heard of the accident,
I felt very, very empty inside.

I wondered what I had missed
those many years ago

in establishing the culture,

or what did I miss in the physical aspects
of the plant?

in order to prevent such a situation.

and it cost him his life.

Growing up, I couldn't have asked
for a better friend.

we tend to want to manage risks

In safety, we cannot afford this mindset,

because, over time, we have learned
we cannot predict the level of risk,

to use our efforts to eliminate hazards
