Source text - English ScienceDaily (Jan. 7, 2011) — Researchers at the University of California, Santa Cruz, have identified a key molecule for establishing blood stem cells in their niche within the bone marrow. The findings, reported in the January issue of Cell Stem Cell, may lead to improvements in the safety and efficiency of bone marrow transplants.
Bone marrow transplants are a type of stem cell therapy used to treat cancers such as lymphoma and leukemia and other blood-related diseases. In a bone marrow transplant, the "active ingredients" are hematopoietic stem cells, which live in the bone marrow and give rise to all the different kinds of mature blood cells. The new study shows that hematopoietic stem cells use a molecule called Robo4 to anchor themselves in the bone marrow.
"Robo4 is a rare molecule that is found only in hematopoietic stem cells and in the endothelial cells of blood vessels," said Camilla Forsberg, an assistant professor of biomolecular engineering in the Baskin School of Engineering at UC Santa Cruz. After earlier work in her lab showed that Robo4 is specific for hematopoietic stem cells, Forsberg set out to discover how it functions.
The discovery that the cells need Robo4 to stay in the bone marrow has potential therapeutic implications. An increasingly common alternative to traditional bone marrow transplants (which require anesthesia for the bone marrow extraction) involves harvesting hematopoietic stem cells from the blood. Repeated injections of drugs are needed to get the stem cells to leave the bone marrow and enter the bloodstream so that they can be collected with a blood draw. A drug that blocks Robo4 could be a safer and more effective way to do this, Forsberg said.
"If we can get specific and efficient inhibition of Robo4, we might be able to mobilize the hematopoietic stem cells to the blood more efficiently," she said. "We're already working on that in the second phase of the project."
Robo4 acts as an adhesion molecule, interacting with other components of the bone marrow to bind the stem cells into their proper niche. Forsberg's lab is trying to find out what molecules bind to Robo4, which could lead to a better understanding of that niche. While other types of stem cells are routinely grown in petri dishes, hematopoietic stem cells are very difficult to grow in the lab. They seem to require the bone marrow environment to function properly, and Forsberg's research might enable researchers to recreate that environment in a petri dish.
Other molecules besides Robo4 are also known to be involved in guiding the localization of hematopoietic stem cells in the bone marrow. Forsberg's results indicate that one of these, called Cxcr4, acts together with Robo4 to retain hematopoietic stem cells in the bone marrow. But the two molecules appear to act through different molecular mechanisms. Inhibition of both molecules may be the best way to achieve efficient mobilization of hematopoietic stem cells, Forsberg said.
Stephanie Smith-Berdan, a research specialist in Forsberg's lab, is first author of the new paper. Coauthors include UCSC researchers Andrew Nguyen, Deena Hassanein, Matthew Zimmer, Fernando Ugarte, and Lindsay Hinck, professor of molecular, cell and developmental biology; Dean Li of the University of Utah; and Jesus Ciriza and Marcos Garcia-Ojeida of UC Merced. Чтобы получить такую кровь, стволовые клетки «принуждают» выйти из костного мозга в кровеносное русло, применяя инъекции определенных сильнодействующих препаратов. «Препарат, блокирующий белок Robo4 может быть значительно более безопасным и эффективным, чем применяемые в настоящее время» – считает Форсберг. –
«Получив специфичный ингибитор белка Robo4, мы сможем управлять процессом выхода стволовых клеток в кровеносное русло значительно эффективнее. Исследования в данном направлении уже ведутся в рамках второй фазы нашего проекта».
Robo4 выступает в роли своеобразного якоря, удерживающего клетку в пределах стволовой ниши в костном мозге. На данный момент исследователи пытаются выяснить, с какими же рецепторами в пределах стволовой ниши взаимодействует белок Robo4. Такие попытки сопряжены со значительными трудностями: в отличие от других типов клеток, стволовые клетки крови не способны к делению вне костного мозга, поэтому перед исследователями стоит нелегкая задача по созданию соответствующих условий in vitro.
Кроме Robo4, существуют и другие белковые структуры, выполняющие сходные с ним функции, например, молекула белка Cxcr4. Но, похоже, механизм действия указанных белков различен, и лишь подавление действия обоих типов молекул обеспечит выход в кровеносное русло стволовых клеток в количестве, необходимом для успешного лечения.

Source text - English Tradename has been shown to produce teratogenic findings (affecting development of fetus or embryo) in animal studies (rats, rabbits) including abnormalities of the limbs. There are no adequate and well-controlled studies in pregnant women and the current recommendation is that Tradename should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus.
There is a literature report of reversible reduction in the ability of human sperm obtained from a limited number of infertile men taking recommended doses of Tradename to bind to and fertilize an ovum in vitro.
When used in pregnancy during the second and third trimesters, drugs that act directly on the renin-angiotensin system can cause injury and even death to the developing fetus. When pregnancy is detected, Tradename should be discontinued as soon as possible.
Therefore, a pregnancy test will be performed in all women of childbearing age at the beginning of the study before taking any study medication and a safe method of contraception should be used during the entire study. Please discuss with the study doctor about contraceptive methods and please inform him/her immediately if, despite this precaution, you become pregnant.

Вплив Препарату® на перебіг вагітності належним чином не вивчався, тому, згідно існуючих рекомендацій, застосування Препарату® під час вагітності припустиме лише в тому випадку, якщо користь від застосування у матері перевершує можливий ризик для плоду.
Опубліковано результати обстеження групи безплідних чоловіків, які приймали Препарат® в рекомендованій дозі. Було виявлено, що застосування Препарату® викликає оборотне зниження здатності сперматозоїдів до запліднення яйцеклітини в лабораторних умовах.

In vitro studies of cisplatin-resistant cell lines have provided some insights into the mechanisms of resistance to this drug. These mechanisms include reduced cellular drug accumulation, cytosolic inactivation of drug, and enhancement of DNA repair. More recent evidence implicates the role of certain genes and proteins in determining the sensitivity of cells to cisplatin. A reduction in the intracellular accumulation of a drug can be due to either impaired influx through the cell membrane or enhanced efflux. A relationship between the amount of cellular cisplatin and sensitivity has been established in various cell lines. The fundamental mechanism of cisplatin transport is, however, not completely understood because of conflicting data in the literature. Gately and Howell proposed a working model in which cisplatin can enter the cell by way of either passive diffusion or a gated channel. The influx through this channel can be affected by various agents, including amphotericin B, dipyridamole, aldehydes, and sodium-potassium adenosine triphosphatase inhibitors, such as ouabain. Thus, these agents could be explored to reverse cisplatin resistance conferred by reduced drug influx. Enhanced efflux of cisplatin has also been observed in some cell lines. Thiol-containing compounds, notably glutathione and metallothioneins, can react with cisplatin intracellularly to inactivate it and prevent binding to DNA. There is ample evidence for this cytosolic inactivation. Increased intracellular levels of glutathione and metallothionein are present in different cisplatin-resistant human carcinoma cell lines. However, not all studies have found the same correlations. Two recent clinical studies demonstrated that tumor cell overexpression of metallothionein correlates with chemoresistance and prognosis in esophageal and urothelial cancers. Preclinical studies have shown that some resistant tumor cell lines exhibit an enhanced ability to repair damaged DNA and that agents that inhibit DNA repair may reverse resistance. This augmented capacity for repair is made possible either by platinum-DNA adduct removal, unscheduled DNA synthesis, repair synthesis, or host cell reactivation of cisplatin-damaged plasmid DNA. Several pharmacologic agents that inhibit DNA repair mechanisms have been studied, including DNA polymerase inhibitors (zidovudine, ganciclovir, and aphidicolin), topoisomerase II inhibitors (etoposide and novobiocin), and methylxanthines (caffeine and pentoxifylline). Other chemotherapeutic agents that exhibit synergism with cisplatin in
experimental systems, including hydroxyurea, 5-FU, cytarabine, and pyrazoloacridine, have been shown to inhibit the repair of DNA-platinum adducts.
Overexpression of various protooncogenes, including c-ras, c-fos, and c-myc, confers cisplatin resistance in vitro via modulation of one or more of the basic mechanisms described above. Recently, nonfunctional p53 protein and loss of DNA mismatch repair were found to confer resistance through a failure to induce apoptosis. Cellular resistance to carboplatin is less well studied.
Because carboplatin and cisplatin share a similar structure, undergo hydrolytic reaction leading to the same active intermediates, lead to the same DNA lesions, and are cross-resistant in most instances, it is assumed that similar mechanisms are involved in carboplatin resistance.

Среди этих механизмов — уменьшение внутриклеточной аккумуляции препарата, цитозольная инактивация и более интенсивная репарация ДНК. Накапливается все больше данных, свидетельствующих о существовании определенных генов и белков, определяющих чувствительность клетки к цисплатину. Уменьшение внутриклеточной аккумуляции препарата может быть связано со снижением поступления препарата через клеточную мембрану, или с его повышенным выходом из клетки. В экспериментах с разными линиями культур тканей была выявлена взаимосвязь между количеством цисплатина, проникающего внутрь клетки, и чувствительностью к нему. Доклинические исследования показали, что некоторые химиорезистентные линии опухолевых тканевых культур демонстрируют повышенную способность к репарации поврежденной ДНК, и что вещества, угнетающие репарацию, способны подавлять химиорезистентность.
